Step 1

MAKE AN APPOINTMENT

SET UP INITIAL APPOINTMENT. We use Jane to manage all of our appointments online. You can also call us at 619-438-4274 to schedule an appointment.

Step 2

DOWNLOAD & PRINT

DOWNLOAD NEW PATIENT FORM. Click button to download, then save pdf. Open & click/type fields (or print/write) filling out as much as possible. Please bring completed form, any lab test(s), & imaging studies that pertain to your condition to your 1st appointment.

Step 3

PROVIDE INSURANCE

PRE-AUTHORIZE YOUR INSURANCE coverage for acupuncture. We accept most insurance carriers, if not we are happy to provide you with a superbill receipt for your reimbursement. Please allow 2-3 business days for verification.

Step 4

PLAN AHEAD

PLAN AHEAD for your 90 minute initial appointment. Wear comfortable loose clothing & eat at least 2 hours prior. Bring STEP 2’s initial paperwork and all diagnostic test results that may pertain to your condition.

Parking available in the rear of the building for patients only, and free street parking also available around the building. Handicap assistance available, please let us know ahead of time to accommodate your needs.
